Note:

• This form must be completed by the person who donated blood, plasma or platelets.
• Donors who received a reward from Hiland Dairy or Culver’s at the time of their donation are not eligible to receive a $5 Dairy Queen gift card.
• Dairy Queen gift cards will be emailed to the email address provided on this form. Please double-check accuracy before submitting.
• The gift card will come from , not a CBCO email address. Please monitor your spam and junk folders.
